2024 Community Health Benefit Fund grant recipients announced

Yampa Valley Medical Center Foundation has announced 18 nonprofits in Routt and Moffat counties, as the recipients of the 2024 grant cycle for the Community Health Benefit Fund. The grants totaled $315,000. The areas of need include health care, mental health, and substance use disorders.

Some examples include the Community Budget Center in Craig which will put their grant dollars towards funding prescription medicines for their clients.

Integrated Community will use its grant fund for the services they provide to immigrants.

2024 Community Health Benefit Fund grant recipients announced

More than $1.5 million invested since 2020 in the areas of access to health care, support of mental health and resources for substance use disorder

 

Steamboat Springs, Colo. (May 21, 2024) – Yampa Valley Medical Center Foundation is pleased to announce 18 nonprofits are recipients of funding from the 2024 grant cycle of the Community Health Benefit Fund.

 

To qualify, organizations have to be an existing 501(c)(3) nonprofit organization, located within Routt and/or Moffat counties and align with at least one of the areas of focus identified in UCHealth Yampa Valley Medical Center’s Community Health Needs Assessment. The areas of need include access to health care, mental health and substance use disorders. The Community Health Benefit Fund committee, consisting of YVMC Foundation board members, hospital trustees and community members, reviewed 23 applications and awarded 18 grants totaling $315,000.

 

The 2024 grant recipients are:

 

  • Better Tomorrow
  • Community Budget Center of Craig
  • The Health Partnership
  • Horizons Specialized Services
  • Integrated Community
  • LiftUp of Routt County
  • Mind Springs Health
  • North Routt Community Charter School
  • Northwest Colorado Health
  • Northwest Colorado Center for Independence
  • Partners for Youth
  • Reaching Everyone Preventing Suicide (REPS)
  • Routt County Crisis Support
  • South Routt School District
  • Steamboat Montessori School
  • Steamboat Springs School District
  • West Routt Fire Protection District
  • Yampa Valley Autism

 

“Since the inception of the Community Health Benefit Fund, the committee has received applications from local nonprofits who are committed to the same things as YVMC and UCHealth – improving lives and the health of the Yampa Valley,” said Kelly Latterman, chair of the Community Health Benefit Fund committee and a member of the Board of Trustees for YVMC. “We are thrilled to once again support these outstanding organizations and their projects. Together, we are making our communities even healthier and stronger than they are today.”

 

Community Budget Center in Craig will put their grant dollars towards funding the prescription medication needs of its clients.

 

“This support will enhance our emergency assistance program and significantly expand our ability to provide immediate and effective aid to those in need. By removing financial barriers to health care, we can ensure more members of our community have access to the critical resources they deserve,” said Kristin Skowronski, board chair of the Center. “This grant empowers us to expand our services and strengthens our commitment to fostering a healthier community. We are profoundly grateful to the Community Health Benefit Fund and Yampa Valley Medical Center Foundation for their generosity.”

 

Integrated Community plans to use its grant funds to enhance the services it provides to immigrants.

 

“We are deeply appreciative of the Community Health Benefit Fund, which is vital to our mission at Integrated Community. This funding will enhance our case management and interpretation services, addressing critical language barriers and significantly increasing health care access,” said Jerry Hernandez, the organization’s executive director. “By improving these essential services, we ensure every community member can receive the care they need, fostering a healthier, more inclusive environment.”

 

Grants from the Community Health Benefit Fund (CHBF), the name given to funds the Foundation received from UCHealth, are split equally between two purposes:

 

  • Annual grant funds for the advancement of health in the communities served by YVMC, including population health initiatives.
  • Special initiatives for the benefit of programs, services and capital projects within or related to YVMC operations and activities.

 

The annual grant funds have been invested to ensure funding is available to support the community and its population health initiatives in perpetuity.

 

The special initiatives funds are earmarked for larger projects that have the opportunity to effect lasting change in the communities served by YVMC.
