Survey for an assessment on Community Health Needs

A Community Health Needs Assessment Survey has been released. It’s done every three years to learn about the most critical health issues facing Moffat and Routt counties.

The survey is a collaboration between Northwest Colorado Health, UCHealth Yampa Valley Medical Center, Memorial Regional Health, The Health Partnership, and the Public Health Departments of Routt and Moffat counties.

The survey is available through Feb. 21.

Your Voice Matters: Help Shape a Healthier Yampa Valley

Routt and Moffat Counties, CO – 5, 2025 – Northwest Colorado Health, UCHealth Yampa Valley Medical Center, Memorial Regional Health, The Health Partnership, and the Public Health departments of Routt and Moffat counties have partnered to launch the 2025 Community Health Needs Assessment (CHNA) survey. Conducted every three years, the CHNA aims to identify the most critical health issues facing Moffat and Routt County residents. A key component of the CHNA is a community-wide survey that is available online from February 1- February 21, 2025 at:

Moffat and Routt County residents 18 and older are encouraged to participate in this anonymous survey, which takes approximately 10 minutes to complete. The survey is available in both English and Spanish and is being distributed through various community channels to maximize participation.

“Administering this survey as a collaboration, we hope to streamline data collection locally. Much of the health data that is collected at the state level can sometimes overshadow what we are seeing in our local communities,” stated Roberta Smith, Public Health Director for Routt County.

A Community Health Needs Assessment (CHNA) is a systematic process used by public health organizations, hospitals, and community stakeholders to evaluate the health needs of a specific population.  It identifies key health concerns, disparities, and resources to guide strategic planning and improve health outcomes.  Key components of the survey focus on healthcare access and utilization, behavioral and social determinants of health, and understanding existing resources and gaps.

The CHNA process helps to prioritize health issues in the community and focuses on areas needing intervention.  In addition, the process informs policy and funding decisions for community health programs.  “The CHNA uses data-driven insights and stories to create meaningful, lasting improvements in health and well-being across the Yampa Valley. We are thrilled to collaborate with our healthcare partners again this year to move this critical work forward, together,” shared Brittney Wilburn, Executive Director at The Health Partnership.

In addition to the survey input, community focus groups are being organized for the end of February to hear additional insight from residents. Participants can choose to enter a drawing for a $250 gift card by providing their name and email after completing the survey
